BOULEVARD MALL BIRTHDAY REWARD OFFICIAL RULES & RESTRICTIONS
- Sponsor: Boulevard Mall.
You understand that you are providing your information to Boulevard Mall, Zamias Serivices Inc., and PlaceWise Media. - Eligibility: NO PURCHASE NECESSARY. MAKING A PURCHASE WILL NOT AFFECT YOUR ENROLLMENT AND ELIGIBILITY. You must present a valid picture ID that verifies your Birthday. Only one reward per person. Birthday Rewards is eligible to those who are 13 years of age or older and are a registered at Boulevard Mall . In addition, eligibility is contingent on receiving and opening emails from Boulevard Mall .
- Timing: The redemption period is bound to the month of the participant’s birthday. Birthday Reward Certificates are sent on or around the first of the month and is good only through the last day of that month.
- How to participate: To participate in Birthday Rewards Program at Boulevard Mall Participants much register for an Profile at https://boulevard-mall.com/. Once clicked the page will redirect to a sign-up form where they will be prompted to fill out information such as first name, last name, email address, birthday, and other information. An email address and birthday information is required for participation in the Boulevard Mall Birthday Rewards program. In addition, participants must agree to receive email communication from Boulevard Mall in order to participate and remain eligible for the program.
- Prizes: Subject to the terms and conditions of these Official Rules, each participant will receive one (1) gift card to be used at Boulevard Mall (gift card subject to the terms, conditions, restrictions, and limitations of the issuer thereof). Each prize is nontransferable, nonassignable, nonnegotiable, and not redeemable for cash or credit. Prize must be accepted as awarded. Each prize is awarded “AS IS” with no warranty, representation, or guarantee, express or implied, in fact or in law, made by Sponsor or for which Sponsor shall be liable, including, without limitation, ANY IMPLIED WARRANTIES OF MERCHANTABILITY, NON-INFRINGEMENT OR FITNESS FOR A PARTICULAR PURPOSE. No prize substitutions, except by Sponsor, who reserves the right (but has no obligation) to substitute a prize (or a component thereof) with another prize of equal or greater value (including cash) if the prize (or a component thereof) is not available for any reason as determined by Sponsor in its sole discretion. Unused components of a prize shall be forfeited and have no redeemable cash value. Participant is solely and fully responsible for any and all costs, fees, taxes, assessments and expenses associated with prize award, redemption, receipt and use, including, without limitation, all federal, state and local taxes on the prize. If applicable, participant will be issued an IRS Form 1099 for the actual retail value of the prize.
- Limitations of liability: No mechanically reproduced or photocopied entries permitted. Neither the Sponsor nor its agencies are responsible for lost, late, misdirected certificates or for lost, interrupted or unavailable satellite, network, server, Internet Service Provider (ISP), website, or other connections availability, accessibility or traffic congestion, miscommunications, failed computer, network, telephone, satellite or cable hardware or software or lines, or technical failure, or jumbled, scrambled, delayed or misdirected transmissions, computer hardware or software malfunctions, failures or difficulties, or other errors of any kind whether human, mechanical, electronic or network. Persons who tamper with or abuse any aspect of this Rewards program or act in violation of the Official Rules, or act in any manner to threaten or abuse or harass any person, or violate these terms of service, as solely determined by the Sponsor, will be disqualified. Neither Sponsor nor its agencies are responsible for any incorrect or inaccurate information whether caused by website users, tampering, hacking, or by any of the programming or equipment associated with or used in this Rewards program.
- Non-eligibility: To receive the Birthday Reward participants MUST have their Birthday information filled out in their profile at Boulevard Mall before their birth month. If the Birthday information is missing from the profile participants will NOT receive the Birthday Reward Certificate. If a new registers after the first of the month for the Birthday Reward program they are not eligible to receive the certificate for that year. They will have to meet the eligibility requirements and they will be eligible to receive the certificate next year. Employees of Zamias Services Inc. and Boulevard Mall Employees its affiliates, subsidiaries, advertising and promotion agencies, wholesale distributors, and individual retail tenants are ineligible.
- Publicity: Except where legally prohibited, by entering participant grants permission for Sponsor and its agencies to use his/her name, photograph, voice and/or likeness, for advertising and/or publicity purposes without further consent or additional compensation.
- Release: By accepting a reward, participant agrees to release and hold Sponsor and its agencies harmless from all losses, damages, rights, claims and actions of any kind resulting from acceptance, possession or use of any reward, including without limitation, personal injuries, death and property damage. By participating in this loyalty program, participant agrees to be bound by all the Official Rules.
- If there is a data-related error with a participant’s information, Boulevard Mall will make every effort to address the issue in a timely manner. Participant must supply Boulevard Mall with all required and requested information in order to resolve it as quickly as possible. If the issue in question is found to be a true data-related error, Boulevard Mall will contact participant to pick up their reward. If there is found to be at the fault of the participant, Boulevard Mall will inform participate of error and how to correct it. At that time it is at the discretion of Boulevard Mall to issue the participant a reward.
